We’ve done a lot of the heavy lifting for you.

  1. Click “Forms”

  1. Then select the appropriate template, if you collect both email and sms then select

If you just want to collect emails and data then select

  1. Click Create the form will open

  2. We have a few housekeeping things to do linking our Terms and Privacy Policy

On the email step, set out limits of your offer or conditions, and highlight and link the terms and privacy policy on this step.

We’ll need to replace the Company Name, Terms and Privacy Policy on the SMS step as well.

  1. Look and feel can be tweaked via the “Theme” section

Then click “Customize”

The Global Styles are colors and fonts, feel free to edit as you see fit.

When you make a change you’ll see the following to revert or to save.

When you’re happy with the look click save

This will prompt you to save your new theme.

The only thing left to set would be a background which can be found on the Form Tab beneath the triggering.

Click “Choose Image” and you’ll be able to update and add a photo

  1. Changing the Coupon Code

On that last step with the Coupon Code, change the Coupon Code to match your existing welcome coupon code. To learn how to set up a unique coupon code see Formtoro - Unique Discount Code Setup.

  1. Once you’re satisfied with the look and feel and have filled out all your questions we want to double check our “Step Names”

All Steps have a name attached to them, these names should be short and descriptive of the steps, they will be used to automatically map your answers into Klaviyo.

  1. Once you’re happy with your “Step Names” we’re all done we just need to connect to Klaviyo and select a list.

  1. Click “Klaviyo” then select your List

(PLEASE ENSURE THIS LIST IS SET TO SINGLE OPT-IN)

  1. Click Next and all your fields will be mapped with your step names by default just click “Activate”

  1. The last thing for us to do is “Publish” the form

Follow the steps

Insert some release notes

Then click “Publish” and you’ll be all set, if the form isn’t already set to live that will be the last step but we prefer to set them live from the main Forms page.
